[Moving Mountains] Netflix type mailers

Does anyone know of a source for mailers similar to the type used by
Netflix?  The mailer would have a removable outer flap for the patron's
mailing address.  On the inner flip, the return library's mailing
address could be printed.  Would also be interested to know if any
libraries have experimented with such a mailing process?
